Hernia diafragmática incarcerada posterior a trauma penetrante de tórax, a propósito de un caso

Resumen


 


La hernia diafragmática se define como la protuberancia del contenido gástrico hacia el tórax producto de un defecto de continuidad en el diafragma. es una entidad reconocida desde la antigüedad, sin embargo, el diagnóstico de esta supone un gran reto para los médicos, ya que por lo general este tipo de lesión se encuentra en sujetos masculinos de mediana edad quienes han sufrido accidentes de tránsito por lo que se da prioriza otras lesiones, sin embargo, la hernia diafragmática puede llegar a ser mortal si no se corrige a tiempo. Dentro de los síntomas comúnmente encontrados esta la disnea, dolor torácico, dolor abdominal, vómitos entre otros que no llegan ha ser del todo específicos. Por este motivo la sospecha clínica, así como los estudios imagenológicos son claves para el diagnóstico, entre ellos la tomografía computarizada destaca como el examen de elección por su alta sensibilidad y especificidad que llega hasta el 87 y 92% correspondientemente, potenciándose con la reconstrucción en tres dimensiones. En cuanto a la resolución de la hernia diafragmática es meramente quirúrgica por lo que la opción de laparotomía y toracotomía constituyen las opciones predilectas, estas tienen como fin reducir los órganos herniados, solventar las funciones digestivas, circulatorias y respiratorias sí como la reparación del defecto en el diafragma para evitar posibles recidivas.
