Nefropatía inducida por contraste. Un caso clínico

Resumen


La nefropatía inducida por contraste (NIC) es una complicación derivada de procedimientos en el ámbito hospitalario, caracterizada por el deterioro agudo o subagudo de la función renal debido a la exposición a un medio de contraste yodado.


Para su diagnóstico se requiere el aumento de 0,5 mg/dl o del 25% posterior a la administración de contraste en comparación a la medición de creatinina sérica pre procedimiento.


Los factores de riesgos dependen de aquellos propios de pacientes y aquellos externos a él. El desarrollo de NIC aumenta la morbimortalidad, días de hospitalización y subsecuentemente gastos para el paciente que la desarrolla.


Se han experimentado una gran cantidad de estrategias preventivas, como son el manitol, furosemida, dopamina y la N-Acetilcisteina, sin embargo, no son alentadoras y algunos no concluyentes según estudios, con el propósito de reducir la carga en términos de morbilidad y mortalidad. Mas sin embargo la evidencia actual soporta el uso de la infusión intravenosa de solución salina a 0,9% antes y después del uso de medio de contraste en dosis ya establecidas.
